Contact your Plan's Benefits Administrator to:
- determine if your Plan Sponsor has elected to use our Member Enrollment utility for eligibility reporting; and, if so,
- obtain your benefit option(s) and enrollment package.
Once you have obtained this information, you may begin the enrollment process.
Please Note: You MUST complete the entry of benefit enrollment information for you and all of your eligible dependents in one entry session. Your data will not be saved until you make your final submission.
